Job summary

Hartford HealthCare Organization is seeking an RN care coordinator to provide expert assessment and planning for patients along the care continuum. You will develop and implement discharge plans within an interdisciplinary team, addressing complex needs and removing barriers to post-acute care access.

You will collaborate with providers to minimize delays, optimize acute care usage, and lead clinical teams to meet daily goals.

Qualifications

• Graduate from an accredited school of nursing.
• Active Registered Nurse license in Connecticut.
• Case management certification is preferred.

Responsibilities

• Develops and executes discharge planning within an interdisciplinary team.
• Identifies and removes barriers to post-acute care access.
• Collaborates with providers to optimize hospital days and prevent delays.
• Leads healthcare teams to ensure daily goals align with the plan of care.
• Supports seamless transitions of care for patients and families.
