Qualifications

• Current BLS certification required.
• Minimum 1 year nursing experience in acute care.

Responsibilities

• Assess psychosocial status and discharge planning.
• Coordinate care across the continuum with the care team.
• Lead patient/family education and care conferences.
• Identify barriers to discharge and resolve delays.
• Document all assessments and care progression accurately.
