Job summary

Wellstar Cobb Hospital is seeking a Care Coordinator RN to assess transitional care needs, coordinate across the continuum, and engage patients and families to meet care goals. The role supports discharge planning, utilization of resources, and collaboration with physicians and care teams.

Minimum 1 year of acute care nursing experience is required; BLS and RN licensure are expected. The position offers day shifts with potential sign-on bonuses and relocation assistance for eligible candidates.

Qualifications

• RN with active Georgia license or multi-state compact desire.
• Minimum 1 year nursing experience in acute care.
• Experience coordinating care across the continuum and with interdisciplinary teams.
• Strong communication and discharge planning skills.

Responsibilities

• Assess psychosocial and functional status and plan transitions.
• Coordinate care progression, discharge planning, and referrals.
• Facilitate patient/family education and care conferences.
• Participate in interdisciplinary rounds and report barriers to discharge.
• Document care plans and maintain up-to-date medical records.
