Qualifications

• Licensed Registered Nurse credentialed from the Connecticut Department of Public Health.
• Three years of acute-care experience in medical/surgical settings.
• Case management, home care, and/or managed care experience.

Responsibilities

• Assess patients and identify risk factors for care coordination.
• Develop discharge planning and monitor plans of care.
• Advocate for patients and educate on health maintenance.
