Longmont United Hospital is seeking an RN Care Coordinator dedicated to advancing patient care and efficient discharge planning. You will lead multidisciplinary collaboration with nursing, physicians, insurers, and post-acute providers to ensure timely, appropriate transitions and optimal outcomes for each patient.

The role emphasizes strong clinical judgment, excellent communication, and advocacy for patients and families.

Qualifications

• RN licensure in applicable state(s) with current license
• Minimum two years acute hospital clinical experience or MSN or Case Management/Education equivalent in lieu of 1 year experience
• BLS certification within 3 months of hire

Responsabilités

• Facilitates multidisciplinary discharge planning with patient and care team
• Coordinates transitions to postacute services based on patient needs
• Completes discharge planning assessment and reassesses as needed
• Identifies readmission risk and implements mitigation strategies
• Oversees discharge plan implementation
• Collaborates with care team to ensure appropriate inpatient resource use
