Qualifications

• Experience with discharge planning and care coordination.

Responsibilities

• Develop multidisciplinary discharge plans with input from the health team and postacute providers.
• Coordinate with care teams and insurers to ensure appropriate postacute transitions.
• Document and reassess discharge planning assessments.
• Identify readmission risks and apply interventions for at least 30 days.
• Oversee discharge plan implementation and progress.
• Collaborate to optimize inpatient resource utilization per guidelines.
