• Oversee progression of care and discharge planning for identified patients
• Coordinate with utilization management, nursing, physicians, and post-acute providers
• Advocate for patients and families and identify barriers to care transitions
• Develop multidisciplinary discharge plans engaging health team and post-acute providers
• Identify readmission risk and implement interventions for at least 30 days
• Evaluate implementation of discharge plans and ensure appropriate inpatient resource use
