Qualifications

• Graduate of an accredited school of nursing.
• Minimum two (2) years of acute hospital clinical experience or a Master's degree in Case Management or Nursing field in lieu of 1 year experience.
• RN license in multiple states as listed in job posting.
• BLS required within 3 months of hiring.

Responsibilities

• Facilitates development of a multidisciplinary discharge plan with health team members, patient, and postacute providers.
• Collaborates with healthcare team and postacute services to ensure timely transitions to appropriate postacute settings.
• Completes and documents a discharge planning assessment and reassesses as needed.
• Identifies readmission risk and implements interventions for at least 30 days.
• Oversees and evaluates discharge plan implementation.
• Collaborates with multidisciplinary team to ensure progression of care using evidence-based guidelines.
