• Graduate of an accredited school of nursing.
• RN license in the states listed or compact equivalent.
• BLS certification within 3 months of hire.

Responsabilidades

• Oversee progression of care and discharge planning for identified patients.
• Coordinate with utilization management, nursing, physicians, insurers, and postacute providers.
• Advocate for patients and families and address barriers to care transitions.
• Develop and document discharge plans and reassess as needed.
• Identify readmission risk and implement mitigation strategies for at least 30 days.
• Oversee and evaluate discharge plan implementation.
• Collaborate with multidisciplinary team to optimize use of inpatient resources.
