Job summary

Life Care Center of Longmont is recruiting an RN Care Manager to coordinate patient care and services, promoting quality care through a collaborative process. You will work with patients, families, the care team, payers, and external entities to ensure safe transitions from the facility to the next care setting in accordance with Life Care standards and regulations.

Qualifications

• Nursing diploma (associates or bachelor).
• Active RN license in current state.
• 1+ year post-acute care or case management preferred.
