Job summary

St. Anthony Hospital in Lakewood, Colorado is seeking an RN Care Coordinator to oversee care progression and discharge planning for identified patients, partnering with utilization management, nursing, physicians, insurers, and post-acute providers to ensure safe transitions.

You will advocate for patients and families, address barriers to care transitions, and apply care coordination principles with a strong commitment to patient advocacy and experience.

Qualifications

• Graduate of an accredited school of nursing
• Minimum two (2) years of acute hospital clinical experience or a Master’s degree in Case Management or Nursing field in lieu of 1 year experience
• RN license in the state(s) covered: Colorado RN:CO or Compact and other listed states
• BLS required within 3 months of hiring
• Bachelor’s Degree in Nursing (BSN) or related healthcare field (preferred)
• At least five (5) years of nursing experience (preferred)
• Certified Case Manager (CCM), Accredited Case Manager (ACMRN), or UM Certification (preferred)

Responsibilities

• Facilitates the development of a multidisciplinary discharge plan with the health team, patient, and postacute providers.
• Collaborates with the healthcare team and postacute service providers to ensure timely transitions to appropriate postacute services.
• Completes and documents a discharge planning assessment and reassesses as needed.
• Identifies readmission risk and implements interventions for at least 30 days.
• Oversees and evaluates the discharge plan implementation.
• Collaborates with the team to ensure progression of care and appropriate inpatient resource utilization.
