Live Well Homecare is seeking a Care Coordinator RN to assess transitional care needs, coordinate across the care continuum, and engage patients and families to support effective discharge planning and care progression.

The role partners with physicians, care teams, and community resources to manage progression, address barriers, and promote timely discharges, while documenting assessments, care plans, and interactions in medical records.

Qualifications

• RN license (single- or multi-state) with BLS certification.
• 1 year of nursing experience in an acute care setting.
• Excellent communication and organizational skills; able to work under pressure in a fast-paced environment.

Responsabilités

• Assess transitional care needs and develop individualized discharge plans.
• Coordinate care across physicians, care teams, and patients to promote timely discharges.
• Coordinate post-acute referrals, evaluate social determinants of health, and ensure insurance/coverage compliance.
• Document assessments, care plans, and interactions in medical records accurately and timely.
• Identify and resolve delays in care, escalate barriers, and optimize resource use.
• Mentor teammates and adhere to organizational policies and standards.
