You will perform utilization reviews, document thoroughly, and collaborate with physicians, payers, and families while maintaining high-quality, cost-effective care.

Required: Bachelor's in Nursing or Social Work, RN/LVN/LPN license, and strong computer/time-management skills; CCM is a plus.

Qualifikationen

• Bachelor's Degree in Nursing or Social Work.
• RN license preferred; LVN/LPN acceptable.
• Two years' clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.
• Ability to read and interpret procedures, manuals.
• Strong math, reasoning and problem-solving skills.

Aufgaben

• Communicate with residents and families about plans of care and treatment.
• Attend stand-up meetings to coordinate with the care team.
• Complete accurate charting and timely paperwork.
• Perform utilization review for appropriate, cost-effective care.
• Coordinate admissions, discharges, and referrals with facilities and payers.
• Monitor resident statuses and adjust care plans as needed.
