PACS seeks a dedicated nursing care professional to communicate plans of care, coordinate with residents, families, and the care team, and oversee admissions and discharges. The role emphasizes high-quality, timely documentation and collaboration with providers and payers.

Ideal candidates hold a nursing degree with RN preferred, LVN/LPN acceptable, and 2+ years of clinical nursing experience, plus knowledge of Medicare/Medicaid.

Formación

• Bachelor’s Degree in Nursing or Social Work.
• RN license preferred; LVN/LPN license acceptable.
• Two years clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.
• Ability to relate positively with residents, families, staff and partners.

Responsabilidades

• Communicate regularly with residents and families about Plans of Care, treatments, and progression.
• Attend morning meetings to facilitate team communication.
• Complete documentation and utilization review activities for timely, cost-effective care.
• Coordinate care with residents, providers, facilities, and third-party payers.
• Oversee admissions, discharges and referrals; organize daily work priorities.
