McPherson Hospital, Inc. is seeking a RMA Patient Navigator to join our care coordination team. Under supervision, you will triage patients, schedule appointments, evaluate health needs, and document care plans in the EHR to ensure timely access to services.

You will work with providers to determine the appropriate clinical course, support walk-ins and phone inquiries, and help coordinate new and same-day appointments while maintaining patient confidentiality and quality outcomes.

Qualifications

• Minimum two years healthcare experience.
• Certified Nursing Assistant / Medication Aide / Medical Assistant.
• Rural health clinic experience preferred.
• Triage experience of at least 1 year preferred.
• CPR certification required.
• Ability to coordinate care across providers.

Responsibilities

• Coordinate and schedule patient appointments on provider schedules.
• Triage patients using established protocols.
• Document symptoms and advice in the EHR.
• Assist with prescription refills per protocol.
• Support care coordination with providers.
• Track no-shows and ensure timely care.
• Set up new patient paperwork.
