Mcpherson Hospital, Inc. is seeking a Registered Nurse Care Manager (RN) to collaborate with the health care team, ensure appropriate admissions and payer compliance, and coordinate medical and supporting services from admission to discharge.

The RN Care Manager will assess, plan, and monitor care, supervise progress with the multidisciplinary team, conduct utilization reviews, and help discharge patients to appropriate levels of care while guiding families to post-hospital resources.

Qualifications

• Must hold an RN license in Kansas.

Responsabilités

• Ensures continuity and coordination of care before, during and after hospitalization, and ensures payer conditions are met.
• Supervises progress with the multidisciplinary team and advises on appropriate clinical interventions per CMS guidelines.
• Performs utilization reviews and assists in Peer-to-Peer processes when needed.
• Monitors readmission rates and LOS; identifies opportunities for improvement.
• Discharges patients to appropriate levels of care and educates patients and families.
• Conducts post-discharge follow-up and provides access to post-hospital resources.
• Serves as patient advocate and participates in policy development and staff education.
