Live Well Homecare is seeking an experienced RN Nurse Navigator Triage to deliver nursing triage services and health advice by phone and in person, assessing patient acuity and coordinating care.

The role emphasizes documentation, patient education, cross-department communication, onboarding support for staff, and ongoing quality improvement to enhance patient satisfaction and care coordination across external providers.

Qualifications

• Graduate from a specialty nursing program.
• Preferred Bachelor's Degree in Nursing.
• Five years of nursing experience, especially in ICU, ED, or Critical Care.
• Proficiency in computer skills and MS Office.

Responsibilities

• Perform patient assessments over the phone, documenting symptoms, history, and contact info.
• Initiate emergency treatment when needed and prioritize urgent calls.
• Provide patient education, schedule appointments, and coordinate care across departments.
• Support onboarding of new staff and ensure effective communication with providers and patients.
• Explain medical conditions, manage patient concerns, and reinforce treatment instructions.
• Ensure accurate documentation and suggest process improvements.
• Assist with external care coordination, including insurance and medical facilities.
