Job summary

TriHealth seeks a Medical Assistant/Medical Home Coordinator to support patient-centered care with strong clinical skills and proactive coordination. The role blends direct patient care in a primary care setting with identifying gaps in care and coordinating necessary follow-ups.

The MA will assist in scheduling, pre-visit planning, and communication with patients through portals. Certification as a CMA/RMA and CPR are required, with EMR and practice management familiarity encouraged.

Qualifications

• Must be certified or registered medical assistant.
• Experience in medical office flow and front desk tasks.
• Ability to coordinate care and communicate with patients.

Responsibilities

• Room patients and perform procedures with proper infection control.
• Document rooming, orders, and patient communications accurately.
• Maintain clinical competency through orientation and ongoing training.
• Support care gap closure through pre-visit planning and care coordination.
• Coordinate messages via patient portals and assist transitions of care.
• Collaborate with physicians and care teams to implement care plans.
