Job summary

Client Growth Resources, Inc. is seeking a Risk Manager / Assistant Director of Nursing (ADON) for a premier long-term care facility in New York. The role leads clinical quality and risk mitigation, with direct responsibility for DOH accident and incident reporting.

The candidate will collaborate with the DON to oversee daily nursing operations, drive high-quality care, and ensure readiness for state surveys, while educating staff on safety and documentation.

Qualifications

• Active, unrestricted RN license in the state of practice.
• Hands-on experience submitting accident and incident reportables to the Department of Health (DOH).
• 3+ years of RN experience in long-term care, skilled nursing, or sub-acute facility.
• Prior experience as ADON, Risk Manager, Quality Improvement Coordinator, or Clinical Supervisor in long-term care.
• Deep understanding of DOH reporting mandates, state surveys, and risk containment.

Responsibilities

• Direct investigation, documentation, and mandatory reporting of all accidents, incidents, and reportable events to DOH.
• Partner with DON to supervise daily nursing operations and maintain high clinical quality.
• Conduct root-cause analyses on clinical incidents and implement corrective action plans.
• Ensure compliance with federal, state, and local DOH regulations and lead survey readiness.
• Educate and train nursing staff on safety protocols, risk reduction, and incident documentation.
