Job summary

Connectly Recruiting is seeking a proactive Risk Manager / Assistant Director of Nursing (ADON) for a premier long-term care facility in New York. The role leads clinical quality and risk mitigation, with direct responsibility for investigating incidents and submitting DOH reportables, in collaboration with the DON to sustain high care standards.

The ideal candidate holds an active RN license, at least 3 years in LTC, and proven ADON or risk management experience.

Qualifications

• Active RN license in the state of practice.
• Hands-on experience submitting accident and incident reportables to DOH.
• 3+ years RN experience in long-term care or sub-acute facility.
• Prior ADON, Risk Manager, QIC, or Clinical Supervisor in LTC.
• Deep understanding of DOH reporting mandates and state surveys.

Responsibilities

• Direct the investigation, documentation, and mandatory reporting of all facility accidents and incidents to DOH.
• Partner with the DON to oversee daily nursing operations and maintain high clinical quality.
• Conduct root-cause analyses on clinical incidents and implement corrective actions.
• Ensure compliance with federal, state, and local DOH regulations and lead survey readiness.
• Educate and train nursing staff on safety protocols and incident documentation.
