Job summary
Top Echelon Software, LLC in New York, NY, seeks a Risk Manager / Assistant Director of Nursing (ADON) to lead clinical quality and risk mitigation in a premier long-term care facility. You will direct investigations, report to the Department of Health, and partner with the DON to uphold high standards of care.
The role requires hands-on DOH reporting experience, 3+ years RN LTC experience, and leadership in clinical teams.
Qualifications
- Active, unrestricted RN license in the state of practice.
- Hands-on experience submitting accident/incident reportables to the Department of Health (DOH).
- 3+ years of RN experience in long-term care, skilled nursing, or sub-acute facility.
- Prior experience as ADON, Risk Manager, Quality Improvement Coordinator, or Clinical Supervisor in LTC.
- Deep understanding of DOH reporting mandates, state surveys, and risk containment.
Responsibilities
- Direct investigation, documentation, and mandatory reporting of all accidents, incidents, and DOH reportables.
- Partner with the Director of Nursing to oversee daily nursing operations and maintain high clinical quality.
- Perform root-cause analyses on clinical incidents and implement corrective actions.
- Ensure compliance with federal, state, and local DOH regulations and lead survey readiness.
- Educate and train nursing staff on safety protocols and incident documentation.
