Top Echelon Software, LLC seeks a proactive Risk Manager / Assistant Director of Nursing (ADON) for a premier long-term care facility in New York. This on-site role leads clinical quality and risk mitigation, with mandatory DOH reporting experience.

Responsibilities include directing investigations, ensuring regulatory compliance, conducting root-cause analyses, and training staff on safety protocols. An RN license and DOH reporting experience are required.

Formación

• Active, unrestricted RN license in the state of practice.
• Hands-on experience submitting accident and incident reportables to the DOH.
• 3+ years RN experience in long-term care or similar setting.
• Prior ADON, Risk Manager, or equivalent leadership in LTC.
• Deep knowledge of DOH reporting mandates and state surveys.

Responsabilidades

• Direct investigation, documentation, and mandatory reporting of all accidents and incidents to the DOH.
• Partner with DON to oversee daily nursing operations and maintain high clinical quality.
• Conduct root-cause analyses and implement corrective action plans.
• Ensure compliance with federal, state, and local DOH regulations; lead survey readiness.
• Educate and train staff on safety, risk reduction, and incident documentation.
