Job summary

AndHealth LLC is seeking a rheumatologist to deliver high-quality care to patients with complex inflammatory arthritis and autoimmune diseases. You will manage a panel, starting with new patient evaluations and progressing toward longitudinal care, in a hybrid model that blends virtual and in-person visits.

You will work with APPs, Pharmacists, RNs, and care coordinators to deliver comprehensive, patient-centered care and participate in program development and partnership activities with CHCs

Qualifications

• Experience with chronic, complex rheumatology patients and autoimmune conditions preferred.
• Comfort practicing in virtual and in-person care environments.
• Strong written and verbal communication skills.
• Ability to lead and mentor APPs and clinical staff.

Responsibilities

• Conduct new patient evaluations and specialist consultations for complex rheumatologic conditions.
• Diagnose, treat, and manage chronic rheumatologic conditions across virtual and in-person care.
• Provide oversight of escalated clinical cases from APPs and pharmacists.
• Develop individualized treatment plans incorporating medical therapy and lifestyle interventions.
• Educate patients on disease processes and long-term management strategies.
• Collaborate in interdisciplinary care planning and care coordination.
