Job summary

AndHealth LLC in Indianapolis, IN is seeking a Rheumatologist to join our growing specialty care team. You will manage a patient panel from early access to longitudinal care, with multidisciplinary support from APPs, Clinical Pharmacists, and Nurses across virtual and in-person settings.

Your role focuses on complex inflammatory arthritis and systemic autoimmune diseases, with emphasis on evidence-based treatment and care coordination to improve outcomes in underserved communities.

Qualifications

• Experience with chronic, complex rheumatology patients and autoimmune conditions.
• Experience mentoring APPs and care team members.
• Comfort delivering care in both virtual and in-person environments.
• Strong written and verbal communication skills.
• Proficient with healthcare technology platforms (EHRs, etc.).

Responsibilities

• Conduct new patient evaluations and specialist consultations for complex rheumatologic and autoimmune conditions.
• Diagnose, treat, and manage chronic rheumatologic conditions across virtual and in-person care environments.
• Provide physician-level review and oversight of escalated clinical cases from APPs and care team partners.
• Develop individualized treatment plans integrating medical therapy, lifestyle interventions, and whole-person care approaches.
• Educate patients on disease processes, treatment pathways, and long-term management strategies.
• Identify co-occurring or contributing conditions impacting diagnosis or outcomes.
• Participate in collaborative case reviews and interdisciplinary care planning.
• Maintain primary responsibility for patient outcomes driven by the care team.
• Ensure compliance with CHC, clinical, and regulatory requirements.
• Collaborate with APPs, Nurses, and Pharmacists to deliver coordinated care.
