Job summary

AndHealth is seeking an Advanced Practice Provider with rheumatology experience to join our onsite care team. You will diagnose, treat, and consult patients, using a collaborative care model to tailor solutions for each individual.

You will coordinate with Health Coaches, order labs, review treatment plans, and help advance our technology-enabled approach to chronic disease care. This is a full-time, onsite position with a focus on outcomes and patient experience.

Qualifications

• Active license to practice in the state of the position.
• NP or PA-C certification required.
• Master’s degree from an accredited program required.
• Rheumatology specialization preferred.

Responsibilities

• Create personalized treatment plans and implement care with patients.
• Review Health Coach cases and provide feedback on plans.
• Assess medications and order labs as needed.
• Oversee patient progress and coordinate care with coaches and providers.
• Collaborate with internal and external providers to ensure access to care.
• Participate in innovating and evaluating AndHealth’s care model.
• Other duties to meet evolving business needs.
