Job summary

AndHealth seeks a Rheumatologist to join a multidisciplinary care team delivering high-quality care to patients with complex rheumatologic conditions. The role starts with a patient panel and moves toward longitudinal management within a hybrid care model combining virtual visits and periodic in-person sessions at Community Health Center partner sites.

You will collaborate with APPs, Clinical Pharmacists, RNs, and health coaches, coordinating care and building provider partnerships.

Qualifications

• MD or DO from an accredited medical school.
• Board Certification in Rheumatology.
• Indiana licensure or willingness to obtain licensure.
• Completion of ACGME Internal Medicine residency and Rheumatology fellowship.
• DEA certification.
• BLS certification.

Responsibilities

• Conduct new patient evaluations and specialist consultations for complex rheumatologic and autoimmune conditions.
• Diagnose and manage chronic rheumatologic diseases in both virtual and in-person settings.
• Provide oversight of escalated cases from APPs and clinical pharmacists.
