Job summary

AndHealth is seeking a mission-driven Rheumatologist to join our growing specialty care team, delivering care to patients with complex inflammatory arthritis and autoimmune conditions.

You will own a patient panel from the start, balancing new evaluations with longitudinal management as referrals grow, supported by APPs and Clinical Pharmacists. This is a hybrid role with care at Community Health Center partner sites and virtual visits.

Qualifications

• Experience with chronic, complex rheumatology patients and autoimmune conditions preferred.
• Strong communication and collaboration with multidisciplinary care teams.
• Ability to practice in virtual and in-person settings; willingness to travel to partner sites as needed.

Responsibilities

• Conduct new patient evaluations and specialist consultations for complex rheumatologic conditions.
• Diagnose, treat, and manage chronic rheumatologic conditions across virtual and in-person care environments.
• Provide oversight of escalated cases and collaborate with APPs, Clinical Pharmacists, and care team partners.
• Develop individualized treatment plans integrating medical therapy and lifestyle interventions.
• Educate patients on disease processes and long-term management strategies.
• Maintain compliance with CHC, clinical, and regulatory requirements.
