Qualifications

• Requires high school diploma; college degree preferred.
• Minimum 2 years of relevant social services experience.
• Ability to manage multiple tasks and meet deadlines in a fast-paced environment.

Responsibilities

• Triage calls for referrals, information, and case manager support.
• Enter referrals into the computer system accurately.
• Complete documentation for Caremanager On Call assistance.
• Assist with booking transportation for consumers with providers.
• Monitor email, voice mail, and information boxes and perform follow-up.
• Maintain and update the Resource Library on community agencies and resources.
• Provide training and support for staff regarding I&R services.
