Job summary

Catholic Charities of the Archdiocese of Chicago is seeking a Case Manager - Resource Hub to assess clients, develop service plans, connect individuals to resources, and advocate for their needs within a partnering network.

You will maintain compliant documentation, collaborate with multidisciplinary teams, and support clients through housing, employment, financial aid, and counseling services to promote positive outcomes and strong community ties.

Qualifications

• Associate degree in a related field and 1–3 years of relevant experience.
• Excellent written and verbal communication skills.
• Strong interpersonal skills and ability to work independently and in a team.

Responsibilities

• Conduct comprehensive client assessments and develop service plans.
• Provide direct support and connect clients to housing, employment, financial aid, and counseling.
• Maintain compliant documentation and advocate for client needs.
• Collaborate with multidisciplinary teams and community partners.
• Engage in ongoing learning and apply feedback to improve performance.
