Job summary

Precious Blood Ministry of Reconciliation in Chicago is seeking a Case Manager to support a caseload of up to fifteen participants. You will guide participants toward productive community roles through employment and education support, life skills training, crisis intervention, advocacy, and resource referrals.

The role emphasizes collaboration with families and partners, data entry, and ongoing progress updates.

Qualifications

• Bachelor’s degree in Human Services field or equivalent experience
• At least 3 years of experience with in-risk youth and trauma awareness
• Experience working with diverse populations and providing advocacy
• Experience documenting case notes and maintaining records
• Knowledge of Chicago neighborhoods Back of the Yards and Englewood preferred
• No pending criminal cases and ability to pass background check
• Driver’s license preferred and Virtus Training completion beneficial

Responsibilities

• Identify and build relationships with at-risk individuals in the target community
• Interview participants to assess needs and arrange services
• Coordinate between internal and external services for participants
• Maintain participation records and advocate for needed services
• Teach job readiness and computer skills in individual and group settings
• Enter case notes and events daily in data systems
• Communicate progress in weekly interdisciplinary meetings and reports
• Develop discharge plans and provide resources for post-program support
• Build partnerships with community resources and hold partners accountable
• Maintain a high level of team engagement as a mandated reporter
