Job summary

Catholic Charities DC is seeking a Case Manager to provide direct management services and coordinate community resources to address client needs.

The role includes interviewing clients, developing case plans, and referring to a range of services. You will collaborate with a multidisciplinary team to support individuals facing housing instability, health challenges and barriers to self-sufficiency.

Qualifications

• Bachelor's degree in human services or related field required.
• Social work licensure in the appropriate jurisdiction is required.
• Professional counseling licensure in appropriate jurisdiction is required.
• Minimum of one-year experience working with at risk clients in a direct service setting.
• Minimum of two years' experience working with clients who have a history of homelessness, with some knowledge of addiction and mental health issues preferred.

Responsibilities

• Interviews clients for direct service needs and develops case plans with written short- and long-term goals.
• Maintains and updates case records, reports and required documentation.
• Collaborates with multidisciplinary teams including legal, medical, housing and mental health professionals.
• Provides referrals for emotional, substance abuse, legal, medical and housing needs.
• Facilitates individual and group meetings and ensures client progress is tracked.
