Responsibilities

• Interviews clients for direct service needs and develops case plans with goals.
• Maintains case records, reports, and documentation across systems.
• Collaborates with substance abuse, legal, medical, mental health and housing providers.
• Facilitates referrals for emotional, housing, employment, and health needs.
• Makes appropriate referrals and maintains statiscal data and compliance.
