Job summary

Catholic Charities of the Archdiocese of Washington is seeking a Case Manager to provide direct management services, identify and coordinate community resources, and deliver supportive services addressing client needs.

You will interview clients, develop case plans, refer as needed, and collaborate with a multidisciplinary team across substance abuse, legal, medical, educational, vocational and mental health fields. A bachelor's in human services with relevant licensure is required.

Qualifications

• Bachelor's degree in human services or related field required.
• Social work licensure in the appropriate jurisdiction is required.
• Professional counseling licensure in appropriate jurisdiction is required.

Responsibilities

• Interviews clients for direct service needs, develop a case plan with the client including written short-term and long-term goals and tasks.
• Reevaluating goals with clients at regular intervals.
• Facilitates individual and group meetings as needed.
• Serves as a member of the multidisciplinary case team.
• Collaborates with other disciplines including substance abuse, legal, medical, educational, vocational, mental health professionals and other service providers.
