Resources for Human Development (RHD) is seeking a Resource Coordinator / Case Manager in Philadelphia, PA. This full-time role involves advocacy, care coordination, and linking participants to housing, health, and community supports to promote recovery and independence.

You will conduct home visits, track progress, and collaborate with shelters and service providers to help participants navigate complex systems while supporting personal goals.

Qualifications

• Bachelor’s degree in a human services field required.
• Valid driver’s license required.
• Strong verbal and written communication skills.
• Ability to work independently and in a multidisciplinary team.
• Trauma-informed, strength-based support approach.

Responsibilities

• Provide advocacy, care coordination, resource linkage, and ongoing support.
• Conduct home visits and community-based meetings.
• Assist participants in navigating benefits and entitlements.
• Document and maintain service records with HIPAA compliance.
• Travel throughout Philadelphia as needed.
