Project HOME is seeking a Residential Services Coordinator 2 to provide person-centered case management and advocacy for residents at Ruth Williams House in Philadelphia. You will support housing stability, wellness, and recovery goals in a compassionate, LGBTQ+-affirming environment.

This full-time role involves coordinating housing, healthcare, and community resources, maintaining documentation, and participating in on-call rotations.

Qualifications

• Experience supporting individuals experiencing homelessness or related populations.
• Availability to work evenings, weekends, and participate in an on-call rotation.

Responsabilités

• Build trusting relationships and provide person-centered support.
• Coordinate housing, healthcare, behavioral health, employment, and education resources.
• Assist with housing applications, documentation, and discharge planning.
• Collaborate with internal teams and community partners to coordinate care.
• Facilitate recovery-focused groups and create an LGBTQ+-affirming environment.
• Maintain timely and accurate case documentation and participate in team activities.
