Project HOME in Philadelphia seeks a Residential Services Coordinator to provide direct support to residents at Philly Home at Girard, a low-barrier housing and respite program. You will use harm reduction and trauma-informed approaches, conduct assessments, develop recovery plans, coordinate services, and advocate for housing stability while promoting dignity and resident choice.

Qualifications

• Education/experience requirements as listed: HS diploma with 4 years related experience, or an Associate/Bachelor's in related fields.
• Experience working with homelessness, behavioral health, substance use, and housing instability.

Responsabilités

• Build trusting relationships with residents and provide person-centered support.
• Meet residents where they are in recovery and housing journeys.
• Utilize harm reduction and trauma-informed approaches in all work.
• Conduct assessments, identify needs, barriers, and goals.
• Develop individualized service and recovery plans to promote stability.
• Coordinate referrals and collaborate with multidisciplinary teams.
