Job summary

Project HOME seeks a Residential Services Coordinator to support residents at Philly Home at Girard in Philadelphia. The role mixes case management, resource coordination, crisis intervention, community engagement, housing stabilization, and recovery support to empower individuals toward stability and self-determination.

Responsibilities include building trust, guiding recovery journeys, using trauma-informed approaches, conducting assessments, and coordinating healthcare, benefits, and housing

Qualifications

• High School Diploma or GED with four years of related experience.
• Associate's Degree with two years of related experience.
• Bachelor's Degree in Social Work, Psychology, Human Services, Sociology, Counseling, or a related field.
• Experience with homelessness, behavioral health challenges, substance use disorders, or housing instability.
• Experience in case management, residential services, recovery support, outreach, or other human services settings.
• Ability to work effectively within a low-barrier and harm reduction environment.
• Availability for on-call, evening, and weekend responsibilities as needed.

Responsibilities

• Build trusting relationships with residents through consistent engagement and person-centered support.
• Meet residents where they are in their recovery and housing journey.
• Utilize harm reduction and trauma-informed approaches when working with residents experiencing complex challenges.
• Conduct assessments and identify resident strengths, needs, barriers, and goals.
• Develop individualized service and recovery plans that promote stability and self-determination.
• Foster a culture of recovery, wellness, dignity, and mutual respect.
• Connect residents to healthcare, behavioral health treatment, substance use treatment, employment, education, benefits, and community resources.
• Assist residents in obtaining identification, benefits, and other necessary documentation.
• Advocate for residents and support access to appropriate services.
• Coordinate referrals and maintain communication with internal and external providers.
• Collaborate with multidisciplinary teams to support positive resident outcomes.
