Job summary

ADAPT Community Network is seeking a Residence Program Specialist to support individuals in a residential facility, promoting dignity, independence, and personalized learning. You will provide direct support, skill-building, and advocacy, ensuring safe, homelike environments and adherence to program goals.

The role involves ADLs support, documentation, crisis response, and collaboration with interdisciplinary teams, with AMAP responsibilities under nursing supervision.

Qualifications

• High School Diploma or GED required.
• AMAP approval at hire or within six months.
• New York State Driver’s License in good standing.
• Ability to read/write/speak English.
• Strong interpersonal and communication skills for working with people we support and families.
• Ability to respond to crisis situations and make sound decisions under pressure.

Responsibilities

• Provides care and supports the people we support to achieve independence and personal development.
• Ensures safety, health, and well-being at all times.
• Teaches activities of daily living as needed and supports social/community living skills.
• Maintains a homelike, respectful atmosphere within the residence.
• Documents progress notes, incidents, and records accurately and timely.
• Responds to crisis situations and medical emergencies and contacts emergency personnel as needed.
