Job summary

Lutheran Social Service of Minnesota in Minnesota seeks a Designated Coordinator to provide direct oversight of residential programs and supervise direct support staff. You will advocate for residents, monitor health, wellness, medications, and budgets in compliance with 245D standards.

This onsite role requires strong leadership, coordination with clinicians, and hands-on support of at least 16 hours weekly.

Qualifications

• Bachelor's degree in a human services field plus 1 year of fulltime direct care experience.
• Associate degree in a human services field plus 2 years of direct care experience.
• Diploma in a human services field plus 3 years of direct care experience.
• 50 hours of education/training in human services and disabilities plus 4 years of direct care experience.

Responsibilities

• Program Oversight: direct responsibility for residential homes serving 4+ individuals, including meals, supplies, and budgets.
• Service Delivery: ensure person-centered outcomes while safeguarding health and safety.
• Team Leadership: guidance and supervision of direct support professionals, including training and performance management.
• Compliance & Quality Assurance: monitor adherence to 245D requirements and organizational policies.
• Collaboration: partner with behavioral analysts, therapists, medical professionals, and community stakeholders.
• Direct Care: provide a minimum of 16 hours in hands-on support, with additional hours as needed.
• Staffing and Shift Coverage: oversight and management of team member schedules and open shift coverage.
• Administrative Duties: managing budgets, documentation, and regulatory communication.

Education

Bachelor's degree in a human services field + 1 year of fulltime direct care experience with individuals with disabilities or adults age 65+

Associate degree in a human services field + 2 years of fulltime direct care experience

Diploma in a human services field from an accredited postsecondary institution + 3 years of fulltime direct care experience

Minimum of 50 hours of education/training in human services and disabilities + 4 years of fulltime direct care experience
