Job summary

Lutheran Social Service of Minnesota is seeking a Designated Coordinator to oversee service delivery in onsite residential settings for 4+ individuals and ensure program quality under Minnesota statutes. You will lead the direct support team, monitor medication administration, and maintain compliant plans and records.

The role emphasizes collaboration with clinicians and community partners to promote safety, wellness, and independence, with some hands-on hours and occasional after-hours coverage

Qualifications

• Bachelor’s degree in a human services field + 1 year of full‑time direct care experience.
• Associate degree + 2 years of direct care experience.
• Diploma + 3 years of direct care experience.
• 50 hours education/training in human services with 4 years of direct care experience.

Responsibilities

• Coordinate service delivery for residential homes serving 4+ individuals.
• Monitor medication administration and health-related records.
• Lead and supervise direct support professionals, provide coaching.
• Ensure compliance with 245D requirements and organizational policies.
• Collaborate with clinical staff and community partners.
• Provide hands-on direct care for a minimum of 16 hours as needed.
