Job summary

Care New England in Rhode Island seeks a Residential Support Specialist to provide person-centered services in residential settings for individuals with behavioral health needs. You will coordinate care, identify gaps in social determinants of health, and connect clients to resources to support recovery and independence.

You will work with the care team to implement individualized plans, facilitate groups, assist with medication supervision, and maintain HIPAA-compliant records.

Qualifications

• High School diploma or GED required; advanced education preferred.
• Valid driver's license and a reliable vehicle.
• Ability to communicate effectively one-on-one and in small groups.
• Knowledge of medication protocols and safety in residential settings.
• Basic computer skills and ability to document in EMR.
• Willingness to travel within the community.
• Bilingual abilities preferred.

Responsibilities

• Provide person-centered support and services within a residential setting.
• Review rules and regulations, issue linens, assign bedrooms, and complete administrative paperwork.
• Serve as a role model to clients, help them solve problems, and ensure program rules are followed.
• Facilitate groups and provide supportive counseling.
• Initiate treatment and advocate for clients within their network of care.
• Collaborate with care team on individualized care plans.
• Teach clients adult daily living skills to assist with recovery and independence.
• Monitor medications and ensure self-administration compliance.
• Maintain accurate records in EPIC and complete daily activity logs.
• Adhere to infection control, HIPAA, and regulatory requirements.
