Connecticut Renaissance, Inc. seeks a dedicated professional to provide case management within a residential treatment program in Connecticut.

You will maintain caseloads per guidelines, screen referrals, coordinate intake, and deliver orientation and basic needs assistance to clients. You will collaborate with clinicians to develop individualized aftercare plans, connect clients to recovery resources, document in the EHR, and participate in meetings and trainings while meeting regulatory

Qualifikationen

• Minimum Associate's Degree in mental health, substance use, human services or criminal justice.
• Experience within the criminal justice and/or behavioral health sectors.
• Experience in Residential Treatment programs a plus.
• Current Connecticut Drug & Alcohol Certification a plus.

Aufgaben

• Maintain a caseload not to exceed established program guidelines and based on needs of individuals actively engaged in services.
• Screen incoming referrals for appropriateness in coordination with the Lead Clinician.
• Input all required documentation in CT Renaissance's EHR accurately and timely.
• Provide program orientation and program handbook to new clients.
• Facilitate connections with recovery resources and community supports, including referrals to self-help programs.
• Collaborate with clients and clinicians on individualized aftercare plans.
• Maintain on-site presence weekdays during 1st shift for at least 80% of the time.
• Participate in required meetings and trainings and complete mandated development activities.
