Job summary

Qualifications

• High School diploma or GED required; advanced education preferred.
• Must have valid driver's license and insured automobile.
• Experience with substance use population preferred.
• Ability to document within electronic medical records (EMR).

Responsibilities

• Provide person-centered support in a residential setting.
• Facilitate groups and supportive counseling.
• Coordinate with care team on individualized care plans.
