Job summary

Lutheran Social Service of Minnesota is seeking a Designated Coordinator to lead residential program delivery for 4+ individuals in Minnesota. You will guide direct support teams, ensure medication administration and health needs are met, and oversee plans and financial records per regulations.

This onsite role emphasizes supervision, collaboration with clinicians and community partners, and hands-on support hours. A supervisory track and required credentials support program quality and safety.

Qualifications

• Bachelor’s degree in a human services field plus 1 year of full-time direct care experience with individuals with disabilities or adults age 65+ is required.
• Associate degree in a human services field plus 2 years of full-time direct care experience is required.
• Diploma in a human services field from an accredited postsecondary institution plus 3 years of full-time direct care experience is required.
• OR a minimum of 50 hours of education/training in human services and disabilities plus 4 years of full-time direct care experience under qualified supervision is required.

Responsibilities

• Provide onsite program oversight for one or more residential homes serving 4+ individuals.
• Coordinate service delivery to ensure person-centered outcomes and health and safety.
• Supervise and coach direct support professionals; manage training and performance.
• Monitor compliance with Minnesota 245D requirements and organizational policies.
• Collaborate with healthcare professionals and community partners to align support.
• Provide a minimum of 16 hours of hands-on direct care weekly, with additional hours as needed.
• Oversee staff schedules and fill open shifts; manage budgeting and regulatory communications.
