Job summary

Oak Hill in Lebanon, CT seeks a Site Manager to lead residential program operations and ensure high-quality supports for participants. You will oversee daily operations, ensure plan-aligned services, and maintain a safe, engaging home environment while supervising DSPs and coordinating with families and professionals.

The role requires strong leadership, collaboration, and compliance with DDS/DCF/ICF and related standards. On-call responsibilities and on-site work are expected.

Qualifications

• Bachelor's degree or equivalent leadership experience in IDD settings.
• Minimum five years of leadership experience in residential services or related field.
• Minimum two years of supervisory/management experience in residential services, healthcare, or related field.

Responsibilities

• Oversee daily operations of an assigned residential location.
• Ensure services align with individual support plans and treatment goals.
• Maintain a safe, clean, and welcoming home environment.
• Monitor program performance, quality indicators, and census goals.
• Provide direct support as needed to meet participant and operational needs.
• Advocate for participants' needs, rights, and goals.
• Coordinate planning meetings (IP, OPS, IDT, IEP).
• Ensure documentation is completed accurately and on time.
• Collaborate with families, guardians, case managers, clinicians, and community partners.
