Qualifications

• Active NJ RN license.

Responsibilities

• Daily wellness checks during home visits.

What You’ll Do

• Daily wellness checks during home visits
• Monitoring chronic conditions
• Reviewing medications & MARs
• Coaching staff on simple health tasks

Key Responsibilities

• Health monitoring & early‑change detection

• Support during hospital/ER transitions

What You Bring

• Active NJ RN license
• ADN or BSN
• 2+ years of nursing experience (community, home care, residential, or IDD a plus)
• Strong assessment & communication skills
• Comfort working independently in the field
• Valid driver’s license & reliable transportation
