Qualifications
- Active NJ RN license.
- ADN or BSN required.
- 2+ years nursing experience in community/home/residential settings.
Responsibilities
- Daily wellness checks during home visits.
- Monitoring chronic conditions and reviewing medications/MARs.
- Coaching staff on health tasks and preparing residents for appointments.
- Documenting health updates and communicating with families and teams.
