Job summary

Continuum of Care in New Haven, CT is seeking a Residential Aide Per Diem to support individuals in daily living activities, promote recovery, and maintain safe living spaces. You will work closely with nursing staff and multidisciplinary teams to ensure high-quality care and client well-being.

Responsibilities include facilitating group activities, supporting medication compliance, and coordinating with families and community resources.

Qualifications

• High School Diploma or equivalent with 1 year healthcare experience
• Licensed CNA or related certification accepted in lieu of experience
• Prior experience facilitating group work
• Experience with mental illness, intellectual disabilities, or addiction
• Knowledge of community resources preferred
• Valid Connecticut Driver’s license with clean record; reliable transport for scattered-site work
• DBT training provided at hiring; CPR/First Aid offered during orientation
• Proficient in Microsoft Office and electronic records management

Responsibilities

• Establish rapport and maintain professional boundaries with clients
• Provide coaching to practice coping strategies
• Assist with daily living, self-care, and medication management
• Support transportation to appointments and community activities
• Maintain safe, clean living spaces and conduct rounds for safety
• Document shift logs and maintain records per policy
• Collaborate with providers, families, and supports
• Respond to crises per agency policy and ensure quality care
