Job summary

Continuum of Care in New Haven, CT seeks a dedicated caregiver to assist individuals with daily living activities, promote independence, and ensure safe, clean living spaces.

You will work with licensed nursing staff, lead group activities, provide transportation, and monitor health concerns, with emphasis on compassionate, professional care.

A high school diploma and CNA certification are preferred; reliable transportation is required, and weekend shift differentials apply.

Qualifications

• High School Diploma or equivalent with at least 1 year in healthcare.
• Licensed CNA or related healthcare certification accepted in lieu of experience.
• Reliable transportation to transport clients between sites as needed.

Responsibilities

• Establish rapport and positive working relationships with clients while maintaining professional ethics and boundaries.
• Provide hands-on support and guidance for activities of daily living.
• Lead group activities and accompany clients to events.
• Administer breathalyzer tests and urinalysis as applicable.
• Document shift logs in electronic records.
• Maintain safe, clean living spaces and perform inspections.
• Respond to crises per agency policy and coordinate with supervising staff.
