Job summary

Continuum of Care in New Haven, CT seeks compassionate staff for 2nd shift (Fri-Sun 3pm-11pm) with weekend differential. The role assists residents with daily routines, prompts for living skills, and participation in groups, while ensuring safety and dignity.

Qualifications

• High School Diploma or equivalent.
• Licensed CNA or other related healthcare certification accepted in lieu of experience.
• Prior experience with people with mental illness, intellectual disability, or addiction.
• CPR/First Aid certification offered as part of orientation.

Responsibilities

• Establish rapport and positive working relationships with clients while maintaining professional ethics.
• Enhance independent living skills by engaging, assessing, and providing various modalities of teaching and assistance.
• Provide hands-on support for activities of daily living, including personal care, housekeeping, nutrition, and budgeting.
• Supervise and document client medications and conduct rounds to ensure safety.
• Participate in group activities, transportation, and community integration.

Education
